Here is the info:
I am gathering information for a presentation to make at an upcoming panel on nutrition among people with thalassemia.  This panel is part of a special NIH-sponsored workshop.

It would help me tremendously if you could respond to a short survey about your experiences with vitamins, supplements and other nutritional issues.
 
The survey should only take 5-10 minutes to complete, and getting feedback is important to being able to present a clear picture of nutrition in the thalassemia community in the U.S..
