Here is the link:
https://www.surveymonkey.com/s/6SQK5DR

Here is the info:
I am gathering information for a presentation to make at an upcoming panel on nutrition among people with thalassemia.  This panel is part of a special NIH-sponsored workshop.

It would help me tremendously if you could respond to a short survey about your experiences with vitamins, supplements and other nutritional issues.

The survey should only take 5-10 minutes to complete, and getting feedback is important to being able to present a clear picture of nutrition in the thalassemia community in the U.S..

The survey can be accessed at:
https://www.surveymonkey.com/s/6SQK5DR

Please complete the survey by August 4.

Thank you very much for your time and participation.

I could not find of it was illegal to share this info...based on that I decided to post it here.

I cannot participate since I am not from USA but I know others can.
