Andy,
As you know we are about to start a second round of ritux for little A. The first round was successful in that it he is free from prednisone - previously he required bi weekly transfusions often dropping to hg of 79 within 2 weeks. After the ritux treatment last summer his transfusion requirement is 20cc/kg every 3 to 4 weeks and most of the time his hg is around 10 before tx. The goal is to reduce his tx requirement to 14-16cc/kg every four weeks and a pretx hg of 10.
A second goal is to reduce his iron levels below 1000 so that we can stop or minimize desferal so that he can grow. Right now he is receiving desferal 5X week, 12hrs a day along with daily exjade. This is a lot of chelation and we don't want it long term. Once his tx requirement is lowered (hopefully the ritux will do that - the doc feels that it will because round one was quite effective even thought it wasn't perfect) we are hoping that maintenance will be easier.
My question is whether IV desferal would be a good idea for little A to drastically bring down his iron levels all at once rather than chasing them down for a long time with all of this chelation. His current iron levels are 1545. We can get his iron levels below 1000 quicker this way and this way we can reduce his desferal requirement to 2 or 3 times a week. With reduced tx requirement (hopefully) maintenance will be easier. I hope that I am making sense. Should we discuss IV desferal with our doctor?

Unless the SQUID had showed iron accumulation in the heart I would not suggest IV desferal. The concern about side effects of chelators when iron levels are already low, would keep me cautious. You don't want to take a chance on compromising his hearing. This is one reason that the hydrazone compounds are so interesting. They showed great potential for safety and effectiveness at low iron levels, and by keeping the iron so low, it helped extend the life of red blood cells that are destroyed by iron oxidation. I would really like to learn more about the direction of research into hydrazone.

Andy,
I am also very eager to get information about hydrazone. Even starch desferal would be helpful in getting iron levels down quicker because it would be present in the body 24hrs a day for the entire week. It would change our quality of life so much. I will let you know what the docs in Oakland say about these two possibilities.

I don't give any advice; I only want to say that I had at the end 5x24hours IV desferal at home. I dont know which pump you use, bt here they use easypump. Every two days a new balloon. It is prepared in the pharmacy. Be sure that they dont increase the volume of desferal when you go to 24hours. I have heard that someone had that problem.

here's a question but bfore that a little contextual introduction :
according to my weight ( 43kg ) i m allowed to take 3 desferal a day .. i m right ??
today when i went to nearest doc's clinic to poke in the canual/Branuala ( watever it called ) they mixed 2 injections in 100ml normal saline . now it is near to finish now . and i want to make last one for today but want to keep it running till time come's to start next day's dose of 3 desferal , (bcoze if it will stop there would b a big chance of needle blockage) .for this purpose i want to make remained 1 desferal ( for today's dose ) in 100ml .. is it wil be right ???... or i can make 2 desferal in 100ml and it will not harm me ??.... please andy and all who is reading this message reply ASAP bcoze 1st 2 desferal are near to finish ... question are :
can i make 1 desferal in 100ml ??
or can i take 4 desferals a day for only today ??? my weight is 43kg ...

You could really take 4 vials every day if you wanted. It is not an extreme amount and with desferal, the amount you can safely take is much higher than the average daily dose that people take. It will be no problem for you to take it today. Lisa sometimes took 4 grams (8 vials) in one day.

when i saw that 1st 2 vials are near to finish, i was confused wat to do , bcoze it was mid-night here and doctor's availability was out-of-question . soo i decided to ask here on forum and got the answer , Thanx Again, Andy :ty ... My probleme is solved .. well, its 02:03 Am here :grin ....

I am so glad that you are getting around the clock IV desferal. This will definitely help get rid of some of the iron from your heart and set you ahead in terms of your chelating. I am hopeful that your ferritin will be below 2000 after this IV chelation session. Most importantly, I hope that the L1 and desferal together are making your heart healthier.
When will you be having your echo and your cardiac mri?

Hey guys,
Little A thanks you all for your congrats after last week's game. So far in the play offs his team has won 1 game (score was 7-0), yesterday the game was tied - so they had 4 over times! and they lost in the very last overtime (score 3 - 2). If they win today they will continue in the play offs so he is pretty excited. I think that his hg may be starting to come down so I hope that he will not burn himself out. I noticed that he was changing shifts a little quicker than usual yesterday, I am suspecting he was burning out a little.

In addition to his regular supplements, keep the carbs flowing and consider some extra B-complex on game days.
Good luck little A! I guess the Syracuse basketball team wasn't the only tean with multiple overtimes this week.

As per your advice he is having pasta for dinner :food
He usually has B complex 50mg - should I give him 2 pills on game days? He has another game after school tomorrow.

Way to go, little A! I am so proud of you.
B vitamins do go through the system quickly, so yes, on game days have him take it twice daily, morning and night with meals. Vitamin D may also be of value.
By the way, my favorite musician, Neil Young, another Canadian, is in his 60's and still rocks. When he tours, he makes sure he eats plenty of carb foods like pasta, to help keep his energy levels up, as he bounces around the stage with his guitar. Hehe, he's also a big hockey fan.

Hey guys,
Update from todays game - the game was tied 2 - 2 - then little A scored a goal for his team in the third period, but the referee did not see it go in so they did not count the goal. The parents and coaches tried to argue it but the referee did not give them the goal - so in the end little A's team lost 3 - 2 and so they are out of the play offs for this season. We have a tournament next week - but little A has rituximab the first day of the tournament so we will see if it is a good idea to let him play.
We have a blood test tomorrow morning - this is his third week post transfusion but he has been playing a lot of hockey and he fought a cold for a few weeks so I am hoping that he is not too low.

Wow ..... Rain of tests ..... i just came back to home after giving blood sample for CBC, LFT and S.Fe ..... what are you asking when i will recieve my reports ?? , mine reporting date is thursday 19 march just a day before the visit ....

hemoglobin in third week is 99. I am glad that he is getting ritux this week. Although his tx requirement has decreased considerably with the first ritux treatment, it would be nice if he could go 4 weeks with 15cc/kg.

You are right, it is not that bad - which I am quite happy about. It shows that ritux is effective for him. The docs in Oakland felt that he was getting too much blood (about 540ml) every 3 weeks and that is too much. If the ritux works well now then he can get 405ml every 4 weeks which will make it easier to get his iron levels down. This way we can reduce, or even eliminate his desferal. Right now it is taking exjade & a lot of desferal to keep his iron levels in control. I hope that it works!
I hope that little Z is doing really well! Is her cold all cleared up? Our doc said that little A had rsv and that is why it lasted so long.

Why did you start on IV Desferal? How're your SF levels now?
I used to take IV desferal of 5g/24 hrs (10 500mg vials) of continuous infusion. I'm now on 2g/10-12 hrs (4 500mg vials) SubQ. I periodically go in for IV infusions of 4g/24hrs. I weigh about the same, too. Generally, you can take more desferal through the IV than the SubQ. I believe for our weight, desferal of around 8g/24 hrs is toxic. Anything below that should be fine but it's not recommended to go THAT high on dosage, especially if you're not much iron overloaded.
What pump are you using, Umair, and are you getting it through the peripheral IV? If yes, how does your skin feel around the site? I usually can't take desferal in peripheral IV for more than 48 hrs at a time. I get a big lump and a lot of pain around the site which takes about a week to fully heal. I also get a lot of irritation around my SubQ site.
In addition to using Solu-Cortef with the Desferal when used Sub-Q, any other suggestions on cream(s) I can use post infusion to help with irritation and to help diffuse desferal more quickly, so, I can use the same site again faster??

My Reports not arrived in Lab yet , hopefully that will arrive at 1900hrs ( its 1700hrs ) , as the reporting time starts from 1900hrs , i m a little confused about the results ( e.g.. wat would b my S.fe) , but i mhopeful that it will below 1500ng , .. Why did you start on IV Desferal?
Buddy , i started IV bcoze i was having symptoms of Congestive heart Failure, soo Andy and all other told me to Start IV desferal bcoze IV desferal remove's the iron from heart more effectively/quickly , soo i started IV contineusly for 63 hours ( almost 3 days ) , ... i wanted to complete 72 hours but i was feeling a little ( very little ) lump on my arm , i felt that it is begining of swelling so i decided stop it just before reaching to the mark of 3 days ... and in those 63 hours i took 12 vials Desferal of 500mg ...
How're your SF levels now?

since Janaury 2009 when my S.fe was 2330ng i started more agressive chelation plan, that is :
i used to take 6 days chelation for 24 hours in gap between 2 Transfussions (12 days ) and 3 vial of Desferal everyday .... last 4-5 times i increased the no# of desferals from 6-8 for the same time ...
Generally, you can take more desferal through the IV than the SubQ. I believe for our weight, desferal of around 8g/24 hrs is toxic. Anything below that should be fine but it's not recommended to go THAT high on dosage, especially if you're not much iron overloaded.
well, i m afraid of desferal's toxicity and side-effects soo m pretty much carefull about it , i first increased the dose of desferal when i was wearing Pump (IM) , i used to take 8 vials and 40ml water for that , i havn't got any lump on the site of desferal but site ( only where the needle was inserted only that area of 2-3cm ) was hardened for couple of days .... i used thigh for that chelation day ... but that wasn't soo disturbing , next day i did the same on second thigh after that i did the same on belly ( on belly i reduced 1 vial of deesferal bcoze i m afraid to have any kind of toxicity on belly :grin but its ok for thighies ) for next time i did the same on arm ( my arms are little skiny so i reduced the dose to 6 vials ) and after all i turned again to 1st thigh and that was pretty ok and ready for chelation ....
What pump are you using, Umair, and are you getting it through the peripheral IV?

once i asked to my doctor that may i mix 3 vials in 10ml water she said "No , it will make solution a little thick and that solution will panitrate very slowly in subq , and you are also complaining of cyst at the site of desferal after having chelation " soo i think more water less thicker solution is the answer for your this quuestion ( i m talking about subQ/IM Desferal ).

i never used any cream or spray on the site of desferal ( before inserting needle ) , but i heard some people used to apply Emla cream before inserting needle and some people used to apply Vitamin-E cream to avoid pigmetation on the desferal site ... here's a thread regarding your this question please check this :

secondly, i want to ask that ,for wat solu-cortef used for ??? and wat is the function of this injection , i heard about this injection to staff (nurse ) once i had fever during Transfussion , she told my bro to bring this injection to treat fever ... please do share if you know anything about this injection ...

I ALWAYS use EMLA cream!!! No Emla, no infusion :bat I used it subq when I was 6 (and did not understand why i could not immediately out of bed, hundreds of time I have heard myself because I forgot to freed the bag from the leg of my bed.) That area is one big fog, but I dont think we even use Emla. I haven't used it very long, because after a few things I got so many lums that I could not bend over anymore.
From the age of 16 untill the age of 20 I used Desferal IV. At the end I had 5x24hrs desferal and on a night in oct 06 I almost died at home because it took my breathe. My body was completely red/rash and lumps. I dont know about the amount, but i have heard from other people with medical background that there lay the problem. So be careful with the concoction.
As for the pump question: I advice everyone to use an Easy-Pump. I still love them. They work like a balloon. You can only use them once. The pharmacist was the witch ;)

Umair, your answers were very carefully thought out and thanks for making sure to answer all questions! I also read all the postings on the thread you referred me to. The postings on that thread were very useful. Here's what I currently do with my DFO (I'll post this here since I noticed no one posted on that thread since Nov 2008 or so):
I mix my 2 g vial (4 500mg vials) in 10 mls. The reason I use 10 mls is because I thought I would not be able to absorb that much fluid. I have heard others using more water to mix the DFO. I may try increasing the volume and see if it helps with irritation and lumps. I am SOO tired of itchiness, redness, and lumps. lumps last at least 4-5 days.
I infuse on my belly only. I have tried my thighs once like 6-7 yrs ago and had a lot of pain. I currently use SofSet MicroSet. One of the members, Jean, on that other thread mentioned she was also using this set and then switched to using Norfolk Clear-View infusion set. I'll see if I can get those. I also have a couple of samples of thalasets at home... they look too fragile.. With SofSet, the issue is exactly what Jean mentioned. It's hard to get through the skin as the design of this set is really bad. It's metal needle at the tip and after small gap (which is the problem) the plastic part starts. the metal needle part is easy to penetrate through skin, but it's the plastic part that really hard to push through the skin. sometimes, I sit on it pushing it through for an entire minute or even more. I have also been told that plastic may also be adding to the irritation. I have been trying to change my infusion sets. I have tried a couple different ones but have not been satisfied with any. some are bigger in gauge and some are too long and stuff...
So, couple things I'll try: a bit more volume to start with. Will try to change inf set as soon as I can. I use 6mm set. So, after doing all that, I may also consider going with 9mm.. Two main concerns I still have but we'll figure them out: 1) would the larger volumes be too much for me and 2)Can I go deeper than I already go wtih 6mm.
I really liked Tariq pointing out how he wears the pump to work and his response to his colleagues :rotfl
For years, I have thought about how to work it out wtih school and work and stuff. I have not found it comfortable wearing to work and school and having people question me about it. I wish there was a pump that didn't make any sound.. Does anyone know of one? I have used Grasby and now I use Crono pump. Crono is quieter, but still would be noticeable.
How's easy pump, Dore? Is it quiet or can you make it quiet? :wink

You know i have learned these tactics with time :grin (Life is the best teacher) You are right, pump is noticeable sometimes but that too can be controlled with the way you wear your clothes (i mean use loose clothing). I use pump by Medis of Italy and it to alarmed too much so i took them to local Medis Dealers and have its alarm busted. For the motor swinging sound, i put a towel like cloth piece under my pump bag and it absorbs most of the sound. (smart heh) There are many funny experiences associated with me roaming around with pump on. I will share them later ;D :crackup
Still i do wish i had something similar to Easy Pump were available here. Oh and BTW i infuse on my upper arm, and its almost painless for me. Thigh infusion is best if you infuse while you sleep.

i think, it is the reason why you're getting lumps on the site of desferal and why the lumps take's too much time to last ... as i mentioed in my last post , that when i aske to my doctor about mixing 3 vials of 500mg in 10ml or 12ml she said no to both , and the reason she told is that , [bgcolor=#fff5ff]less water thicker solution and thicker solution mean's hard to panitrate and it will take long time to panitrate and it will remain like a lump/cyst ... [/bgcolor] soo she told me to use 5ml for per vial of 500mg ...
another thing i red in your post you mentioned you're using 6mm needlle , buddy , i think its a little shorter to reach the subQ layer for you, if your not soo skiny .... i think it will be better to try 8mm or 10mm and in the end 12mm needle ... i remember in 1995-1999 when nobody had any know how to insert the needle and how deep it should be layed ( beneath the skin ) , usualy docotrs pricked the need and remained it in skin and within a few hour i star feeling pain itchiness redness and cyst at site , soo i think getting a little long needle would be helpful for you ....
only to share : a few ( not more than 3-4 times) Times i used to take 8 injection in 48 hours and 40ml of water at thigh ... no reaction, no pain, no lumps, no redness and no itthcines .... Thanx GOD

You know i have learned these tactics with time :grin (Life is the best teacher) You are right, pump is noticeable sometimes but that too can be controlled with the way you wear your clothes (i mean use loose clothing). I use pump by Medis of Italy and it to alarmed too much so i took them to local Medis Dealers and have its alarm busted. For the motor swinging sound, i put a towel like cloth piece under my pump bag and it absorbs most of the sound. (smart heh) There are many funny experiences associated with me roaming around with pump on. I will share them later ;D :crackup
Still i do wish i had something similar to Easy Pump were available here. Oh and BTW i infuse on my upper arm, and its almost painless for me. Thigh infusion is best if you infuse while you sleep.
