I am back to Karachi. What started as a full of fun and masti trip turned out to be a scary one. While on our way to lahore in train, one of our companion thal Naveed had severe nose bleeding which scared the crap out of us. Doctor from another center who was also in same train helped us out. Well then, bleeding stopped somehow, and we reached lahore and were directly taken for inauguration session of conference. That inauguration turned out to be my only participation in conference as next day early morning Naveed's condition got worse and he got severe abdomen pain, diarrhea and again nose bleeding. We rushed him to hospital where he was given saline to overcome dehydration. And there they found out his BP has dropped to critical level (50/20) and he was going in Septicemia. So he was shifted to CCU and since no one from his family was here, we patients had to attend 24 hour and take care of everything talking to doctors etc. He was given necessary treatment as prescribed by Dr. Javeria and his BP came in normal range after 15 hours and he started talking and eating after 24 hours. He is still admitted there but much better. Now the only problem is his platelet count and for that, he has been already transfused 7 units of FFP factor but he still needs 5 more for now for which we are all trying to arrange blood donors. We also called his family members next day and his bro and mum flew to Lahore in the middle of night. Now they and two of my thal buddies who extended their stay there are attending him. Please pray things work out well for him.
The reason Naveed visited lahore was he was already facing issue for the past 6 months and he has continuous fever since then and all sort of lab investigation has already been done but no reason for illness was yet diagnosed. So he was trying to show all his reports to Dr Javeria and other docs for opinion and also he has been recommended splenectomy and he also wanted to discuss it with docs. He had history of nose bleeds thats why we were not that worried after his nose bleed episode in train after we reached Lahore but then we found out things were severe this time.
Thal patients who were registered in the thal center of same hospital Naveed was in, were really really supportive and I was truly amazed to see there co-operation level and also the staff and doctors were really kind and supportive. Also guys from Thal Federation of Pakistan were there all the time to help us take quick steps bypassing official requirements to speed things up.
I will post more updates as they arrive.
@Umair, thank you for supporting and being there for me while we were in stress, it really means a lot.
Keep praying all of you,
Tariq.

Tariq,
You and your friends who have watched out for Naveed are thalpals in the truest sense. You may have missed most of the conference but you have done something far more important by helping Naveed stay alive.
In life you will have guypals and galpals but nothing beats the thalpals!

The nosebleeds indicate a chronic low platelet condition. If possible, have this info passed to the doctors to make sure they are aware of the various causes, as it may help to narrow down why Naveed has this ongoing problem. See
http://www.mayoclinic.com/health/thrombocytopenia/DS00691/DSECTION=causes
# Idiopathic thrombocytopenic purpura (ITP), a condition in which your immune system mistakenly identifies platelets as a threat and forms antibodies that attack them.
# Other autoimmune diseases, such as lupus or rheumatoid arthritis, which may lead to destruction of platelets due to a malfunctioning immune system.
# Blood poisoning from severe bacterial infections (bacteremia), which may lead to destruction of platelets.
# Thrombotic thrombocytopenic purpura (TTP), a rare, life-threatening condition that occurs when small blood clots suddenly form throughout your body, using up large numbers of platelets. TTP sometimes happens as a result of a genetic deficiency, but more often the cause is unknown. In some cases, it may be associated with infection or a chronic illness.
# Hemolytic uremic syndrome, another rare disorder that causes a sharp drop in platelets, destruction of red blood cells and impairment of kidney function. Sometimes, this can occur in association with a bacterial Escherichia coli (E. coli) infection, such as may be acquired from eating raw or undercooked meat (often hamburger).
Investigation for bacterial and viral illness, along with any of the other possibilities should be done.

As far as I know his case, he immediately needs his spleenectomy done. But the continuous fever is making things complicated as doc suggested he should not go for spleen removal until fever settles down for couple of weeks, as that could also complicate things. Right now, he is getting frequent transfusions and low platelet counts and also his DFO was stopped by doc due to chronic infection symptoms. And also he is diabetic :( I am sure once spleenectomy is done, thing will get better for him. He is a pretty strong fellow with will power I haven't seen in many people. The way he stood up in the matter of hours from such critical condition is not easy.
Lets hope for the best
Tariq.

I heard about Naveed while i was chatting with Umair on MSN this evening. it was sad to hear. i also met both Tariq and Umair in the conference at PC Hotel, Lahore.
My prayers are with Naveed. Umer is with Naveed at the hospital and since Dr. Jovaria is looking after him, it is somewhat relaxing that he is in safe hands. she is a very nice and proficient doctor. But the unknown reason of this illness is alarming. I guess it might be due to enlarged sepleen or food poisoning.... The biggest issue they are facing at the moment is that they are new in Lahore and that is actually making problem in getting the blood arrangements since they don't know anyone around here. I sent a blood donation request to some of my friends and acquaintances here in lahore. hope that they contact Umer (Naveed's friend) soon.
May Allah bless him health soon.

Guys,
Naveed is admitted again in HDU surgical Liaquat National Hospital. He was taken to emergency with severe abdomen pain. Ultrasound and CT scan disclosed that his large intestine has inflammation and infection and he has to go through surgery. But after consulting with surgeon and other doctors, it was decided to control the infection with medicines as he is in no shape for surgery. Right now he is on dextrose and insulin as all sort of solid or liquid food intake is stopped. He has already been started antibiotics for infection and after one day of treatment, pain has reduced a lot and he was looking better than before. I was with him for few hours and just got back.
Doctors and surgeon are experienced there and they have good hands-on experience in handling thals, so I think Naveed is in good hands for now.
Naveed needs your prayer people, I am really worried about him.
I will update again.
Tariq.

Update:
I was with him this evening. Pain is almost in control, but he is still under monitoring. No other problems with him so far. Lets hope he is shifted to normal ward as environment in HDU is making him feel more sick as all sort of surgical cases come in HDU. Also, its his third day without food intake.

Praying and hoping that he shifts to the ward and then to the home, with all cure through medicines only...

Naveed was discharged this evening. He is doing fine, slowly moving back to normal things. Having light solid meals. Doctors are planning his spleenectomy after a month or two, and they have started preparing for it giving some pre-surgery medicines to improve
his immune system.
Thank you for your prayers,
Tariq.
