IV desferal is usually given for no more than two weeks at a time, unless the patient is in heart failure. In between IV sessions, pumping should continue at least 5 times per week. The patient's dose of deferasirox is too low. With those iron numbers, the patient should be on high dose of at least 40 mg/kg daily. For this patient, it should be 1600-2000 mg daily, split into two doses to maximize the benefit. It is critical that aggressive chelation be undertaken for at least two years.
