In October, my employee and friend, Chuck, who is a thal minor, underwent a bone marrow transplant for lymphoma. Over the past few days, things have grown dramatically worse, as his lung function has gotten very bad and he was put on a ventilator. His oxygen level is only 86% with the ventilator. His wife sent me this message yesterday.
On the 16th, he was admitted into Strong Hospital for shortness of breath. They believe he has pneumonia and a build up of fluid on his lungs. He is once again on the bone marrow transplant floor, and he is fighting for his life. We are trying to be positive, but it is difficult. I saw the CT scan, and the doc. said she had never seen anyone with such a compromised lung system look and sound as good as he does.
Chuck has a wife and three children. Please pray for his recovery.

This morning's update.
Chuck is on a ventilator to keep his lungs moving so he can fight the
infection. The next 24 - 48 hours are critical to see if he can regain his
strength. He is on the Pulmonary intensive care unit, where they can help
him the most. We will keep you updated. thanks. dd

I heard from Chuck's wife this morning. Nothing has changed much, which can be seen as a positive, since it isn't worse.
He hasn't changed significantly. They were able to do a bronchoscopy
Wednesday and we should have the results today so they can fight the
infection with the right anti-biotics. They've been treating him with a
combination of anti-s. and with the results of the culture they got during
the bronk. they should be able to home in on it more efficiently.
He still looks great; great color, etc. but he's still under and will stay
that way until there's a big improvement.

On Friday, the update was not good. The doctors were afraid that Chuck had Diffuse Alveolar Hemorrhage, which is an often fatal side effect of bone marrow transplant. I did not want to share this until the doctors were sure. Sigh of relief. I just received this message.
OK, it seems it is not dah (which is NOT a good thing to have). Now they are leaning more toward Idiopathic Pneumonia Syndrome. Not as harsh a recovery time, and it's being treated. They've lowered his ox. dependency on the machine to 40%, and lowered the PEEP too, so hopefully, he will continue to do well on this regimen. I'll keep you updated! Thanks, Andy.
Idiopathic Pneumonia Syndrome in BMT patients appears to be related to the pre-BMT conditioning with radiation treatments and can also be harsh. However, DAH is worse and Chuck is making some progress in regaining his oxygen independence.
Let's pray that Chuck continues to improve and gets through this soon.

Chuck continues to show some improvement. I just got the latest update.
Good news today. Chuck's PEEP has been lowered to 8, and he's tolerating it well. They may wake him up soon, but I don't know when. It'll be a big transition, so I'm a little wary, but I'm excited too.
I didn't go see him today, b/c the weather has been dangerous.

I'll keep you posted. And for those who don't really "get" snow, the reality is what she said. The weather is dangerous. It's been snowing for the past 24 hours and the roads are a mess. Even my cat is cursing the weather.

Do you have any further updates on Chuck? Have they woken him up yet? I pray that things will improve and that they will wake him up soon.

There continues to be slow improvement and Chuck has been taken off the meds that have kept him under for two weeks, in anticipation of removing the ventilator. It's slow and there is much to do just to get him breathing on his own again, but honestly, this could have easily gone the other way, so today we are thankful and have hope that tomorrow will be brighter.
The family thanks everyone for their prayers and wishes.

Chuck is now conscious but still on the ventilator. A tracheostomy was done to minimize the damage done by the tube to the throat and vocal chords. Once he can be weaned off the ventilator he will be sent back to the bone marrow transplant floor because is his immuno-compromised. Very slow progress and his lungs are still being treated for both pneumonia and DAH. Hopefully he can get off the ventilator soon.

Wednesday, Chuck spent 8 hours off the ventilator. Thursday he was scheduled for 12 hours breathing on his own. In addition, he is talking. the doctors are amazed because there was concern about damage to his vocal chords from the ventilator tube. He is no longer in the intensive care ward but he is still being treated for pneumonia and DAH. He will eventually be transferred to the bone marrow ward. Khalifa and I conferenced in with Dr Polchi on messenger recently and Dr Polchi told me that when BMT is used for treating lymphoma that some host vs graft disease is necessary for the lymphoma to be wiped out. So, Chuck will have that to look forward to. It's a long road after BMT as we saw with Abdulwahab and it's a dangerous road at times. At least for the moment, things are looking up.

Chuck has been off the ventilator for a few days now and has been breathing on his own with no trouble. He has been eating and has a good appetite, which is good because he lost 50 pounds. His muscle mass is gone and he can't move much on his own, so he will need a lot of physical therapy. He won't be going home soon because he needs to rebuild some muscle and also strengthen his immune system before he is exposed to the outside world. He has been moved to the bone marrow ward so that conditions are basically more sterile. When his wife stopped by yesterday, the sense of relief was obvious. She's been through so much the past few months but is holding together really well, but she can use a break.

that true this kind of operation need to wait and slowley treatment i didn't replay before because i know it's very defecult to say any thing because i was in the same setuation and i wait almost 13 month to get my son Abdulwahab back to me .now i can say to Chuck CONGRATOLATION and be a way a way from chunk food also an oily food just boiled food and some easy exersise at home ,,,,,

After having a couple good weeks off the ventilator and starting some physical therapy, Chuck has had a setback. He has pneumonia again. He isn't on the ventilator but on a ventilator mask this time. I heard from his wife this morning and he is slightly improved today. Let's pray this is just a temporary setback.

Frankly, I advised him against it and told him to get a second opinion but he never did. I thought he is too old and also that BMT was overkill for what he had. Stubborn guy.
Even though I can go to a conference and speak to the best bone marrow transplant doctors in the world, and have very close familiarity with BMT cases, and do know a thing or two, people outside the thal world often think I am just blowing hot air. I can't do anything about the people who think that if you're not a doctor, you can't know anything. I've had that happen here too. We had a thal minor make a hasty exit when she learned I was not a doctor. Her loss, because I know far more about thal minor than 99.9% of the doctors on earth. We can only help those who are willing to listen.

It happens with me too,i can't claim i know so much,but i want to share whatever knowledge i have,but people never listen,we can only hope for their best.

Andy,
You definitely know more about thal than most general practitioners - you even know more than most doctors that treat thal. I think that those of us who have learned from you and incorporated it into our lives have benefitted greatly - and those who haven't have missed out on an opportunity to live a healthier and better life. You advise patients on a daily basis, you save lives and you give us support - I think that you are more than a doctor. Maybe the thal minor who left will go back taking her iron pills and living her anemic, iron overloaded life - her loss entirely.
I really hope that the outcome of Chuck's bmt is good in the end. His wife and children need him. I will keep his family in my prayers.

Today's news is not good. Chuck's lung function is not good and he has to be put back on the ventilator. Now the doctors believe it is DAH, which is life threatening.

Well, I learned today that they did not put Chuck back on the respirator yet, but have stayed with the ventilator mask for now. I guess this is good news somewhat. The problem is his lungs are not functioning well and this is likely due to side effects from radiation and chemo before the BMT. These side effects can be deadly, so it is still a very tenuous situation, at best. We will continue to pray that stubbornness will prevail, and that Chuck will continue to tell death to take a hike.

The last news I got on Chuck was on Monday and was not good. Chuck was put back on the ventilator on Saturday because his lungs are not functioning well enough on their own. I am very worried.

There's a slight improvement as the week has gone along and the drugs are working faster on his lungs this time. Let's hope he's not on the ventilator much longer.

I was able to visit Chuck today. I hadn't seen him since January because I had a cold that wouldn't leave and by the time it did, he was back on the ventilator. Chuck's family and friends were gathered in a family room in the bone marrow ward, just down the hall from his room, for an Easter buffet meal. Chuck was brought into the room in a large adjustable hospital chair where he was able to sit up in a reclining position. For what he's been through, Chuck looked better than I thought he would. He was lucid and coherent and we talked for quite awhile. He also ate quite a bit over a 2 hour period. It was good to see him getting real food again. He is able to raise his arms up and use his hands which is a big improvement. We both agreed that his stubbornness was responsible for him still being alive. It's still too early to tell if his lungs will recover this time but he is doing better and they're being slower about lowering his prednisone dose this time. If all continues to improve, he will most likely be in a rehab home for a short period until he is strong enough to come home.
My biggest impression was that Chuck has improved quite a bit since he was taken off the ventilator the second time. His arms and hands were stronger than I expected and his mental awareness and ability to communicate were both good. If his lungs recover, I think he'll be back home within a month.

Thanks to Umair for reminding me to do an update.
Chuck has been steadily improving and today he was released from the hospital to a rehab center in the same hospital, so technically, he is no longer hospitalized. This is wonderful news and if all goes well, once he is strong enough and able to walk on is own, he will be released to go home. He is very much looking forward to this, as he has been back in the hospital since January.
Let's hope the worst has now passed.

Chuck's going home today. He can't walk yet because his body got so weak while he was on the ventilator but he's doing physical therapy and coming along. I hope he can stay healthy this time.

How is Chuck doing now?

Chuck's doing well. He has been having physical therapy and is walking with a walker but hopes to be rid of it within a month. His lungs have been doing good, so he may finally be out of danger from that problem. His white cells are back to normal, which is great. Strangely, so far his hematocrit is only 28, even though the BMT should have cured his thal minor. Maybe it will improve as time passes. Overall, he is recovering well.

It's been awhile since I updated about Chuck. I see him every week and he has made good progress. In fact, he was deemed well enough for surgery and had his gallbladder removed today. Things went well and he should be released tomorrow. He continues to have low hemoglobin, in the 8-9 range, but hopefully this will eventually improve. All in all, he's doing pretty good. His recovery from the lung problems was amazing and his lungs have healed well. It's good to see an older adult make it through a bone marrow transplant and gives much hope to those who explore this for their children. The techniques continue to be refined and BMT's become safer every day.

Thanks Andy for sharing the wonderful news, I am happy to know that he is doing well. He had passed actually through a lot of challenges ( i was too worried about his age) but he seems to be a fighter

Andy, how is Chuck doing? Did he recover from the gallbladder operation?

Chuck actually came into my shop this week (wearing a mask) and is slowly recovering. He was moving a bit slower but he's recovering from surgery.

Chuck is a fighter. So glad he is recovering and doing fine.

It is with a very heavy heart that I announce that Chuck has passed on Friday, October 12 at the age of 60. The BMT had cured the lymphoma, but it did so much damage to his lungs and organs that he never fully recovered. The latest bout of pneumonia was too much for his body.
Chuck had returned to work a few hours a week for me. This is heartbreaking news, Prayers are requested for his wife and three children.

I am feeling shame in asking or saying at this moment, but Andy, from what I know lymphoma is of lymph nodes, and I too have enlarged lymph nodes and recently I always have problem in nose in breathing, more like sneezing/cold. Should I be concerned? Can lymphoma happen to me?

This is very heartbreaking Andy. It feels like we went through the entire experience with him and we all feel the loss. Sending prayers to his family and to you - as you have had to say good bye to so many people close to you recently.

Anyone can get lymphoma. My 18 year old neighbor just finished chemo for it and is doing well. I really wish Chuck had stayed with the traditional treatment, rather than allow an irresponsible doctor do the BMT so he could have a big pay day.
