Hi my wife is thalassemia major on regular transfusion with iron chelation. Present ferrtin is around 3500.her heart is moderate and liver is severe in T2* mri.
2 D echo is 55 %. Shown to physician and chest physician also both have said it's due to anexity and hypervenation .she became breathless after waking small distance or by climbing steps.Her hb is maintain on 8.5 to 10 gms.pls any one having same problems ..help me

Yes almost every patient would feel breathlessness with those statistics. She should maintain pre-Hb of atleast 9, better 9.5 or above, and most importantly, intensive chelation should be pursued to lower ferritin levels. This is extremely important, patient of her age and mentioned facts should religiously follow chelation plan to lower ferritin. Antioxidants should be taken to provide protection from ferritin damage. Milkthistle should be taken to help liver. Wheatgrass, calcium, vitamin D, healthy diet and moderate exercise altogether improve health and stamina but everything is secondary to successful chelation.

I agree completely with Waleed. She shouldn't let he Hb drop below 9.5 and she needs to clean up the moderate iron load, which does affect heart function and will cause breathlessness. It is not due to anxiety, although the breathlessness can be cause for anxiety. She needs to get the iron out. She'll be much better off once she does.

Hi,
Mere watching LVEF % is not worth, a cardiologist must check for diastolic dysfunction.
Low calcium also causes breathlessness.
High VitD and Vit B12 or Bcomp can also help.

Then, pls maintain pre bt hb 10+, stay away from direct sun heat as it is a damn hot summer. Take nutritional diet. maintain Sodium , potassium levels which easily gets depleted in summer and with chelation.

Normal B-12 is not enough for thals. The level should be at the high end of normal.
